Updated 06/25/26 · By grantmaking.aiThe AI Incident Database (AIID) is a centralized, systematized repository of incidents where artificial intelligence systems have caused or nearly caused harm. Modelled on incident databases in aviation and computer security, it records and organizes these events so that stakeholders can study patterns in AI failures and use that knowledge to prevent or mitigate similar bad outcomes in the future.
